About Greg Goh

Physiotherapist.
Practice owner.
Structural systems architect.

Building what I wish had existed when I needed it.

I spent over a decade running an allied health practice and learning — the hard way — that growth without structure doesn’t create freedom. It creates dependency. Practice OS is the answer I built to that problem, from the inside of a practice that needed a different kind of operating architecture.

For the first several years of running my practice, I did what most practice owners do — I made myself available for everything. Clinical decisions, operational questions, staff issues, scheduling problems. Everything came through me, and I told myself that was just what leadership looked like at this stage.

What I eventually realised was that I hadn’t built a practice. I had built a dependency. Every new clinician we hired, every service we added, every step toward growth made me more central to the operation, not less. The practice was scaling. My workload was scaling with it.

“The structural question I started asking wasn’t ‘how do I work harder?’ It was ‘why does my practice need me this involved — and what would have to change for it not to?'”

The answer wasn’t a better morning routine. It wasn’t a new mindset. It was structure. The organisation had grown past the point where informal systems and owner availability could hold it together — but nobody had designed anything to replace those things.

I started working on that problem deliberately. Mapping the decision flows. Designing leadership layers. Defining what each role owned and what it didn’t. Installing the meeting rhythms and KPI dashboards that meant the practice could produce visibility without me doing the checking.

Practice OS is the distilled result of that work. Not a framework borrowed from corporate consulting — a structural install designed specifically for the allied health context, built from real practice problems and refined through direct implementation. It exists because I needed it, and because I’ve spent enough time inside other practices to know the pattern repeats.

The work now

What I do and who I work with

I work with a small number of allied health practice owners each quarter to install the operational architecture that removes owner dependency. The engagement is founder-led, structured, and limited — because the quality of the install matters more than the volume of engagements.

Alongside the install work, I offer a monthly structured advisory arrangement for practice owners who are earlier in the growth curve and not yet ready for a full Practice OS engagement.

I am also a clinic partner within the Healthia Group network, which means my thinking about structural operations is tested daily inside a real practice — not just applied to other people’s.

Practice OS — 8-Week Install

The primary engagement. We design and install the full operational architecture across eight structured weeks. Limited to a small number of practices per quarter to protect delivery quality.

1:1 Structured Advisory

Monthly advisory for practice owners working through early-stage operational and leadership challenges. Structured, not open-ended — each session is built around your specific execution priorities.

“The content has been great — particularly the 1:1 sessions, which gave me the ability to take the general topics and reflect on how they relate to me personally and the business, and then get direct guidance on how to apply them in the next stages.” Greg Hoekman, Managing Director — Tanunda Physio & Health
