The four structural pillars
Practice OS installs four interconnected layers. They work as a system – none are optional, and each builds on the one before it. The sequence matters as much as the content.
Pillar One
Direction
Before anything else can be installed, the practice needs clear organisational intent. This pillar defines where the practice is going, what the owner’s role within it is, and what constraints are preventing forward movement. The owner shifts from reactive to intentional, with a clear mandate for how they spend their time and attention.
Without Direction, every other layer is built on ambiguous foundations. Teams can’t execute toward a direction that hasn’t been defined. Leaders can’t own priorities that don’t exist.
Pillar Two
Ownership
This is where the leadership architecture gets designed. The future-state org structure is built, leadership layer roles are defined, and the owner’s direct reports are reduced to a manageable number, which is typically five or less. KPI ownership is redirected from the owner to the leaders closest to the work.
Most scaling practices have the right people. What they lack is a structure that tells those people what they’re responsible for and empowering them to decide. Ownership installs that structure.
Pillar Three
Execution
Structure without a rhythm to run it is just a document. Execution installs the operational cadence – the weekly leadership meeting, the standard agenda, the decision authority framework that tells every leader what they can decide and what they escalate. Action tracking is embedded so follow-through becomes visible without the owner chasing it.
After this pillar, decisions start resolving at the right level. The leadership team begins operating with predictable rhythm and work in alignment towards the objectives that move the practice forward.

Signals
The final layer gives the practice operational visibility – simple KPI dashboards, performance indicators, and early warning signals that mean the owner no longer relies on gut feel alone. Leaders own their metrics. Performance is visible to those accountable for it. Problems are identified and resolved with minimal owner involvement.
Signals is the layer that makes the system self-correcting. It turns the owner from an operational hub into a strategic one.

Map the current org structure. Identify direct reports, decision escalation patterns, and the owner’s time distribution. Establish the bottleneck baseline.
Leadership Layer Design
Design the future-state org structure. Reduce owner direct reports. Define the leadership layer roles — what each leader owns, decides, and is accountable for.
Decision Authority Mapping
Define what each leader decides independently versus what escalates. Set financial and operational thresholds. Remove the ambiguity that forces decisions upward.
Meeting Cadence Install
Install the weekly leadership meeting – standard agenda, KPI review rhythm, action tracking. The first milestone payment falls here, at point of demonstrated value.
KPI Ownership & Signals
Define 3–5 KPIs per leadership role. Assign clear ownership. Build the simple dashboard that gives the practice performance visibility without owner involvement in data collection.
Accountability & Follow-Through
Clarify RACI where needed. Align consequences to roles. Remove undefined responsibilities – the pattern where accountability exists in name only.
Growth Capacity Unlock
Convert freed owner time into a forward-looking growth focus. Identify remaining capacity constraints. Set hiring and expansion triggers based on the new structural baseline.
Stress Test & Lock-In

How is this different from business coaching?
Coaching works on the owner. Practice OS works on the practice. We are not facilitating reflection, building mindset, or providing accountability. We are designing and installing structural architecture – org charts, decision frameworks, leadership roles, meeting systems, KPI dashboards – directly into how your practice operates. The output is a set of artefacts that run after we leave, not insights that require ongoing reinforcement.
What if I don’t have a leadership team yet?
This is one of the most common situations we work within. Most practices at the 8–15 clinician stage have capable people but no defined leadership layer. Practice OS designs that layer as part of the install, identifying who in your existing team can step into a leadership role, what those roles look like, and what authority sits within them. We work with what you have, not what you wish you had.
How much of my time does the install require?
Each of the eight weekly sessions requires approximately 90 minutes of your time. There is also a short review period between sessions where you are implementing the previous week’s artefact with your team. The total owner time commitment is typically 15–20 hours across the 8 weeks with the direct outcome of freeing 4–6 hours per week permanently on the other side of the install.
What happens after the 8 weeks?
Week 8 closes with a full structural lock-in review and a 90-day sustain plan – a structured document that tells you exactly what to monitor, what to protect, and what to adjust as the system settles into your practice. The install is designed to be self-sustaining. If you want ongoing structural advisory support after completion, a separate 1:1 mentoring arrangement can be discussed at that point.
How do I know if my practice is ready?
That’s what the diagnostic call is for. It’s a 45-minute conversation where we look at your current structure, your direct reports, your escalation patterns, and your owner time distribution. At the end of the call you’ll have a clear picture of whether Practice OS is the right fit for your practice right now, and if it isn’t, what would need to change first.
What if my practice isn’t at the right stage for a full install yet?
Practice OS is designed for practices with 8–20 clinicians where structural complexity is already the problem. If you’re earlier than that, or you want to work through specific operational and leadership questions before committing to a full install, a structured monthly advisory arrangement may be a better starting point. One 60-minute session per month, built around your specific execution priorities.
